Who is eligible to enroll in WVU Employee Benefits?

Benefits-eligible Employees

Must work a minimum of 9 months of a 12 month period and be scheduled to work a minimum of 1,040 hours during the 12 month period. This includes classified, non-classified, and Faculty Equivalents/Academic Professionals (FE/AP). Examples include employees who work at least 37.5 hours per week over nine months, or employees who work at least 20 hours per week over 12 months. Faculty members who work .72 Full Time Equivalency (FTE) or greater in a nine-month period are also benefits-eligible.

Transferring Employees

If an employee transfers from one participating West Virginia state agency to another in the middle of a plan year without a lapse in employment, the employee may continue with PEIA coverage uninterrupted. Such a transfer does not create an initial enrollment period, so the employee will not have the option to make changes to health or life insurance.

An employee may change health plans only if the transfer moves the employee out of the enrollment area of a plan so that accessing care is unreasonable. Since the PEIA PPB plan has an unlimited enrollment area, an employee with this plan is not permitted to transfer out of it during the plan year, even if the employee moves.

Mid-Year Plan Changes

The only time an employee can change plans during the plan year is when a qualifying life event occurs or when the employee moves out of the enrollment area of their managed care plan.


How do I enroll or make changes to my WVU Employee Benefits?

An employee may complete benefit enrollment

  • During their new hire enrollment period which includes the calendar month in which employee is hired in a benefits-eligible position and the following two calendar months.
    (Example: Hire date is August 16; New hire enrollment period is August 1 through October 31.)
  • In the calendar month in which a qualifying life event takes place and the following two calendar months. Changes are effective the first day of the month following signed change form.
  • During the annual Open Enrollment period (usually during the month of April each year). Changes made during Open Enrollment are effective on July 1 of that year.

How to Enroll

New employees may enroll in their employee benefits by processing remotely or by attending a New Employee Benefits Session.

Making a Change

Current employees with a qualifying life event may enroll in or make changes to their health and life insurance online through PEIA’s Manage My Benefits website at www.wvpeia.com or by completing a PEIA Change In Status form. Change in Status forms must completed and returned to the WVU Benefits Office along with any required supporting documentation.

To make changes to your Mountaineer Flexible Benefits, please complete and return an FBMC Change In Status form within 60 days of your qualifying life event.

    Eligible Dependents

    Employee’s legal spouse.

    Employee’s biological or adopted children, stepchildren, or other children for whom you are the court-appointed guardian under age 26.


When will my insurance coverage begin?

Health insurance coverage will become effective the first day of the month following the date of enrollment. If you enroll before your start date, coverage will begin the first day of the month following your first day of active employment.

Employees have the month in which a qualifying life event takes place and the following two months to make the change. Approved changes in status will be made effective the 1st of the month following the date of the event or receipt of your change in status request, whichever is later.

(Example: If you were married in June, but completed a change in status request to add your spouse in July, your spouse’s coverage would begin August 1st.)

What is a qualifying life event?

Under the IRS Code Section 125 rules, plan participants may only make changes to their insurance coverage during open enrollment each year or when certain qualifying life events occur.

When a Qualifying Life Event occurs, you have the month of the event, plus two following months to submit your change in status to PEIA. You may make changes to your PEIA health and/or life insurance coverage online through PEIA’s Manage My Benefits website at www.wvpeia.com or you may complete a PEIA Change In Status Form and return to WVU Employee Benefits office along with any required supporting documentation.

Changes to Mountaineer Flexible Benefits must be made within 60 days of the date of event by completing a Mountaineer Flex Benefits Enrollment/Change-in-Status Form. Return all paper forms to the WVU Benefits Office along with any required supporting documentation. (Campus mail to PO Box 6640)

Qualifying life events include

Beginning of spouse's or dependent's benefit plan A letter from the employer stating the termination or retirement date, what coverage was lost, and what dependents were covered
End of spouse's or dependent's employment A letter from the insurance carrier indicating the change in insurance coverage, the effective date of the change and the dependents covered
Significant change in health coverage due to spouse's or dependent's employment A letter from your insurance carrier indicating the change in insurance coverage, the effective date of the change, and the dependents covered
Unpaid leave of absence bu policyholder, spouse, or dependent A letter from your, your spouse's, or your dependent's personnel office stating the date that you, your spouse, or your dependent went on unpaid leave or returned from unpaid leave
Change from full-time to part-time employment or vice versa for policyholder, spouse, or dependent A letter from your, your spouse's, or your dependent's employer stating the previous hours worked and the new hours worked as well as the effective date of the change

Dependent Verification Documentation

To add dependents to your insurance coverage, you must include documentation that verifies each individual as an eligible dependent. Please include copies of the following when adding a dependent to your plan:

  • a marriage certificate (to add a spouse);
  • birth certificates (to add children);
  • adoptions papers, if applicable;
  • copies of dependent VISAs (if dependents are foreign nationals and do not have US social security numbers).

How to Enroll your new dependent in PEIA health and/or dependent life insurance

  • Option 1: Online Enrollment

    Go to: www.wvpeia.com

    Click the Manage My Benefits button at the top of the page

    Log in or Register a new account

    To add new dependents to coverage, click “Add a New Dependent” and follow the instructions

    Be sure to Finalize your changes!

    If adding dependents, please upload copies of required dependent eligibility documentation or submit to the WVU Benefits Office with the policyholder’s name and last 4 digits of SSN written in top right hand corner.

  • Option 2: Paper Form

    Complete and submit the PEIA Change In Status Form, along with any required documentation, to the WVU Benefits Office: One Waterfront Place – 2nd Floor, Campus Mail: PO Box 6640

  • Changing Mountaineer Flexible Benefits

    Mt. Flex Benefits Change in Status form

    Complete and submit the Mt. Flex Benefits Change in Status form along with any required documentation to the WVU Benefits Office.

Required Documentation

To add dependents, PEIA requires “Dependent Eligibility Documentation:”

  • Spouse: Copy of Marriage Certificate
  • Children: Copy of Birth Certificate
  • Adopted Children: Copy of Adoption papers or Court Documents
  • Foreign Nationals: Include copies of VISAs in addition to marriage and birth certificates for any dependents who are citizens of a foreign country
  • Please include the policyholder’s name and last 4 digits of SSN on ALL paper documents submitted to the WVU Benefits Office.

Please use the PEIA Required Documentation Guide as a reference when making changes to your coverage as a result of a qualifying life event.

A Note About Beneficiaries

Important life events such as marriage, divorce, death, and/or birth of a child are good times to re-evaluate your beneficiaries. Please work with your life insurance and retirement vendors directly to update your beneficiary information. PEIA life insurance beneficiaries may be updated online through PEIA’s Manage My Benefits website at www.wvpeia.com.


What do I do if I haven’t received my health insurance card?

Please allow 2 – 4 weeks from your effective date of coverage to receive your health insurance card from PEIA. If you need schedule a doctor’s appointment or use your health insurance benefits prior to receiving your insurance card, please contact PEIA directly at 1.888.680.7342 to request a temporary ID card.


Do I get cards for dental or vision insurance?

No. WVU’s dental and vision providers do not distribute ID cards. ID cards for these plans aren’t necessary when seeking services from an in-network provider.

How to Print your Delta Dental ID Card

  • Go to www.deltadentalins.com
  • Log in to Online Services with your username and password. (If you don’t already have a username or password, click “Register Today” link to complete the quick registration process.)
  • Once you’ve logged in, click the “Eligibility & Benefits” tab.
  • Select “Print ID Card” on the left-hand side of the page. (If you do not see this option, in some instances you may also need to click on the “Eligibility & Benefits” link on the left-hand side of the page before you have the option to select “Print an ID Card.”)
  • Click “Print.”
